Communication is the payer's way to ask for a missing document and ours to answer. It is also what carries admission, treatment and discharge updates — NHCX has no separate API for those.

The sandbox dummy payer does not reply to Communication. Outgoing messages are delivered and logged; an incoming reply needs a real payer.
